The aim is to provide people with continuous care to improve their health and well-being and to make healthcare more affordable.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">PMBs are a feature of the Medical Schemes Act, in terms of which medical schemes have to cover the costs related to the diagnosis, treatment and care of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>any emergency medical condition;<\/li>\n\n\n\n<li>a limited set of\u00a0<a href=\"https:\/\/www.medicalschemes.co.za\/download\/2032\/prescribed-minimum-benefits\/18097\/pmblstofallconditions.xlsx\" target=\"_blank\" rel=\"noopener\">271 medical conditions<\/a>\u00a0(defined in the Diagnosis Treatment Pairs); and<\/li>\n\n\n\n<li><a href=\"https:\/\/www.medicalschemes.co.za\/publications\/#2009-2152-wpfd-chronic-disease-list-algorithms\" target=\"_blank\" rel=\"noopener\">26 chronic conditions<\/a>\u00a0(defined in the Chronic Disease List).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">When deciding whether a condition is a PMB, the doctor should only look at the symptoms and not at any other factors, such as how the injury or condition was contracted. This approach is called diagnosis-based. Once the diagnosis has been made, the appropriate treatment and care is decided upon as well as where the patient should receive the treatment (at a hospital, as an outpatient or at a doctor\u2019s rooms).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>PMB<\/strong>&nbsp;Definitions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The legislation governing the provision of the prescribed minimum benefits (PMBs) is contained in the regulations enacted under the Medical Schemes Act, 1998 (Act No. 131 of 1998).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In respect of some of the diagnosis treatment pairs (DTPs), medical scheme beneficiaries find it difficult to know their entitlements in advance, while medical schemes interpret these benefits differently, resulting in a lack of uniformity of benefit entitlements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The benefit definition project is coordinated by the CMS, with the aim to define the PMB package; and to guide the interpretation of the PMB provisions by relevant stakeholders.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The guidelines are based on evidence of clinical and cost effectiveness, taking into consideration affordability constraints and financial viability of medical schemes in South Africa.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Which conditions are covered?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The Regulations to the Medical Schemes Act in Annexure A provide a long list of conditions identified as Prescribed Minimum Benefits. The list is in the form of Diagnosis and Treatment Pairs (DTPs).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A DTP links a specific diagnosis to a treatment and therefore broadly indicates how each of the approximately 271 PMB conditions should be treated. The treatment and care of PMB conditions should be based on healthcare that has proven to work best, taking affordability into consideration. Should there be a disagreement about the treatment of a specific case, the standards (also called practice and protocols) in force in the public sector will be applied.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The treatment and care of some of the conditions included in the DTP may include chronic medicine, e.g. HIV-infection and menopausal management. In these cases, the public sector protocols will also apply to the chronic medication.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here is an example of a DTP as it appears in the Medical Schemes Act:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><th>Code<\/th><th>Diagnosis<\/th><th>Treatment<\/th><\/tr><\/thead><tbody><tr><td>109A<\/td><td>Vertebral dislocations\/fractures, open or closed with injury to spinal cord<\/td><td>Repair\/reconstruction; medical management; inpatient rehabilitation up to two months<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">The 271 conditions that qualify for PMB cover are diagnosis-specific and include a range of ailments that can be divided into 15 broad categories:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><th>PMB Category<\/th><th>Example<\/th><\/tr><\/thead><tbody><tr><td>Brain and nervous system<\/td><td>Stroke<\/td><\/tr><tr><td>Eye<\/td><td>Glaucoma<\/td><\/tr><tr><td>Ear, nose, mouth and throat<\/td><td>Cancer of oral cavity, pharynx, nose, ear, and larynx<\/td><\/tr><tr><td>Respiratory system<\/td><td>Pneumonia<\/td><\/tr><tr><td>Heart and vasculature (blood vessels)<\/td><td>Heart attacks<\/td><\/tr><tr><td>Gastro-intestinal system<\/td><td>Appendicitis<\/td><\/tr><tr><td>Liver, pancreas and spleen<\/td><td>Gallstones with cholecystitis<\/td><\/tr><tr><td>Musculoskeletal system (muscles and bones); Trauma NOS<\/td><td>Fracture of the hip<\/td><\/tr><tr><td>Skin and breast<\/td><td>Treatable breast cancer<\/td><\/tr><tr><td>Endocrine, metabolic and nutritional<\/td><td>Disorders of the parathyroid gland<\/td><\/tr><tr><td>Urinary and male genital system<\/td><td>End-stage kidney disease<\/td><\/tr><tr><td>Female reproductive system<\/td><td>Cancer of the cervix, ovaries and uterus<\/td><\/tr><tr><td>Pregnancy and childbirth<\/td><td>Antenatal and obstetric care requiring hospitalisation, including delivery<\/td><\/tr><tr><td>Haematological, infectious and miscellaneous systemic conditions<\/td><td>HIV\/Aids and TB<\/td><\/tr><tr><td>Mental illness<\/td><td>Schizophrenia<\/td><\/tr><tr><td>Chronic conditions<\/td><td>Asthma, diabetes, epilepsy, hypothyroidism, schizophrenia, glaucoma, hypertension<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>No exclusions<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Medical schemes often have a list of conditions \u2013 such as cosmetic surgery \u2013 for which they will not pay, or circumstances \u2013 such as travel costs and examinations for insurance purposes \u2013 under which a member has no cover. These are called exclusions. Exclusions, however, do not apply to PMBs. If you contract septicaemia after cosmetic surgery, for example, your scheme has to provide healthcare cover for the septicaemia part because septicaemia is a PMB. (Cosmetic surgery remains an exclusion.) PMBs are concerned about the diagnosis; it doesn\u2019t matter how you got the condition.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Download the PMB list with ICD10 codes\u00a0<a href=\"https:\/\/www.medicalschemes.co.za\/download\/2028\/icd10-codings\/24945\/2013-pmb-icd-10-coded-list-distribution\">here<\/a><br><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>What happens in an emergency?<\/strong><\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">An emergency medical condition means the sudden and, at the time, unexpected onset of a health condition that requires immediate medical treatment and\/or an operation. If the treatment is not available, the emergency could result in weakened bodily functions, serious and lasting damage to organs, limbs or other body parts, or even death.<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">In an emergency it is not always possible to diagnose the condition before admitting the patient for treatment. However, if doctors suspect that the patient suffers from a condition that is covered by PMBs, the medical scheme has to approve treatment. Schemes may request that the diagnosis be confirmed with supporting evidence within a reasonable period of time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Which chronic diseases are covered?<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The Chronic Disease List (CDL) specifies medication and treatment for the&nbsp;<a href=\"https:\/\/www.medicalschemes.co.za\/publications\/#2009-2152-wpfd-chronic-disease-list-algorithms\" target=\"_blank\" rel=\"noopener\">26 chronic conditions<\/a>&nbsp;that are covered in this section of the PMBs:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Chronic renal disease<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Addison\u2019s disease<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Asthma<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Bronchiectasis<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cardiac failure<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cardiomyopathy<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Chronic obstructive pulmonary disorder<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Coronary artery disease<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Crohn\u2019s disease<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Diabetes insipidus<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Diabetes mellitus types 1 &amp; 2<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dysrhythmias<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Epilepsy<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Bipolar Mood Disorder<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hypothyroidism<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hypertension<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Glaucoma<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Haemophilia<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ulcerative colitis<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Systemic lupus erythematosus<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Schizophrenia<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Rheumatoid arthritis<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Parkinson\u2019s disease<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hyperlipidaemia<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Multiple sclerosis<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To manage risk and ensure appropriate standards of healthcare, so-called treatment algorithms were developed for the CDL conditions. <a href=\"https:\/\/www.medicalschemes.co.za\/publications\/#2009-2152-wpfd-chronic-disease-list-algorithms\" data-type=\"link\" data-id=\"https:\/\/www.medicalschemes.co.za\/publications\/#2009-2152-wpfd-chronic-disease-list-algorithms\">The algorithms,<\/a> which have been published in the Government Gazette, can be regarded as benchmarks, or minimum standards, for treatment. This means that the treatment your medical scheme must provide for may not be inferior to the algorithms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you have one of the&nbsp;<a href=\"https:\/\/www.medicalschemes.co.za\/publications\/#2009-2152-wpfd-chronic-disease-list-algorithms\" target=\"_blank\" rel=\"noopener\">26 listed chronic diseases<\/a>, your medical scheme not only has to cover medication, but also doctors\u2019 consultations and tests related to your condition. The scheme may make use of protocols, formularies (lists of specified medicines) and Designated Service Providers (DSPs) to manage this benefit.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">CMS&nbsp;<strong>can<\/strong>&nbsp;<strong>help<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">PMBs can be a rather complicated subject and your medical scheme might not be able to answer all your questions. Sometimes, your medical scheme may be reluctant to provide you with the cover you are entitled to for a PMB condition and you need someone to champion your cause.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Do not despair. The Council for Medical Schemes (CMS) was established to supervise medical schemes in South Africa.
